Gastric Sleeve

HOW IT WORKS:

  • Using small incisions in the upper abdomen, the majority of the left part of the stomach is removed.
  • The remaining part of the stomach is a narrow tube called a sleeve.
  • This makes the stomach smaller and pushes the food through quicker.

BENEFITS:

  • The reduced capacity of the stomach restricts how much one can eat.
  • The food moving through faster results in gut hormonal changes which contribute significantly to weight loss as well as improvement in other health conditions such as diabetes and high blood pressure.
  • The removed part of the stomach is responsible for a lot of the appetite driving hormones and hence there is significant reduction in appetite post-op.

Gastric Bypass

HOW IT WORKS:

  • The stomach is sectioned off, leaving a small pouch to serve as a new stomach
  • The small bowel is divided and joined to the small stomach pouch
  • This allows the majority of your stomach and some of the small intestine to be bypassed

BENEFITS:

  • The reduced capacity of the stomach restricts how much one can eat.
  • Bypassing the section of the GI tract results in gut hormonal changes which contribute significantly to weight loss as well as improvement in other health conditions such as diabetes and high blood pressure.

Dr Marjan (Jane) Ghadiri is an experienced female general surgeon, actively involved in both the elective and emergency care of patients. Whilst she maintains a breadth of practice that includes hernias and skin lesions, she is highly specialised in the treatment of gastro-oesophageal diseases and morbid obesity.

Originally a University of Queensland medical graduate in 1997, she completed her General Surgical training at the Alfred Hospital in Melbourne in 2010. Her post-fellowship training in Upper Gastro-intestinal and Obesity surgery at Monash Medical Centre paved the way for her ongoing clinical interest in this area. She has been an active and valued member of the staff at Monash and Epworth Health until her recent relocation.

Am I a Suitable Candidate for a Weight Loss Surgery?

Generally, patients who undertake bariatric surgery must have either a body mass index (BMI) equal to or greater than 40 if they have no additional health conditions related to obesity, or a BMI of equal to or greater than 35 if they have additional health conditions related to obesity. In certain situations bariatric surgery may be appropriate in those with a BMI of 30-34, and this will be assessed on a case by case basis by Dr Ghadiri. It is important that surgery is only undertaken when the likely benefits outweigh the potential risks.

Procedure Risks

The following is a list of potential complications from bariatric surgery, whilst only 2% of patients have any adverse effects at all and the majority of those are minor, it is important to be aware of the potential issues and to discuss any concerns with your surgeon.

Some of the common risks associated with bariatric surgery include:

Infection: As with any surgical procedure, there is a risk of infection at the incision site or within the abdomen.

Bleeding: Excessive bleeding during or after surgery is a potential risk.

Blood Clots: Surgery increases the risk of blood clots forming in the legs (deep vein thrombosis) or traveling to the lungs (pulmonary embolism).

Anastomotic complications: Some bariatric procedures such as gastric bypass involve joins between sections of bowel and stomach. These joins can develop complications including bleeding, stricturing (narrowing) or leakage. Whilst these are rare they can be serious and require urgent care.

Staple-line leakage: Some bariatric procedures such as sleeve gastrectomy involve the use of staples and where these have been used there is a risk of incomplete healing, leading to leakage from the staple-line which occurs in 1/200 cases and is a serious complication requiring urgent care.
Digestive Issues: Bariatric surgery can cause changes in the digestive system, leading to issues like dumping syndrome, diarrhoea, or nutrient deficiencies.

Gallstones: Rapid weight loss after surgery can increase the risk of gallstone formation.

Ulcers: Stomach ulcers can develop in some patients after bariatric surgery.

Hernias: Incisional or internal hernias may occur, necessitating further surgical intervention.

Nutritional Deficiencies: Malabsorption of nutrients can lead to deficiencies in vitamins and minerals, such as vitamin D, B12, iron, calcium, and others.

Gastroesophageal Reflux Disease (GORD): Some bariatric procedures including sleeve gastrectomy can bring about new GORD symptoms or worsen existing GORD; whilst others can be used to treat known GORD.

Weight Regain: Physiologic weight regain over the years is a recognised phenomenon in all people, including those who have had weight loss surgery; Appropriate follow-up, education and support can help minimise and post-pone such regain for many years.

Psychological and Emotional Changes: Bariatric surgery can have a significant impact on a person’s mental and emotional well-being, which may require counseling or support.

Functional Issues: Bariatric surgery can alter the way the gut functions, leading to unexpected changes which may require specific lifestyle and dietary modifications in addition to those experienced by the majority.

It’s important to note that these risks can vary depending on the type of bariatric surgery and the patient’s individual health and medical history. The benefits of bariatric surgery often outweigh the risks for individuals with severe obesity, but thorough evaluation and discussion with a healthcare provider are crucial to making an informed decision about the procedure. Additionally, strict adherence to post-operative dietary and lifestyle recommendations can help minimize some of these risks.
